Report findings while the information is still fresh, to improve how we plan and perform work next time.

How

Examples of questions to ask:

  • Was the task carried out as expected?
  • Has the workplace been restored?
  • Are there any experience, incident or risk observation that need to be reported?
  • What can be done to improve this task or similar tasks in the future?
  • Were instructions and drawings relevant?
  • Were resources, tools and information sufficient?
  • Was education / training sufficient for effective execution?
  • Was enough time set aside for the task so that it could be completed within the planned time?
  • Were the work routines effective?
